Integrating Advanced Practice Providers into Team-Based Care: A Qualitative Examination of APP, Physician, and Healthcare Leader Perspectives.

Abstract
Background. Team-based care has been shown to improve both provider and patient outcomes across the spectrum. The use of this collaborative care model has surged secondary to the rising prevalence of chronic diseases, physician shortages, burnout, and the transition to new payer models that emphasize quality of care. Advanced practice providers (APPs) play a key role in team-based care models, and research shows they improve care quality, patient access, and provider satisfaction.
Study Purpose. The purpose of the study is to identify the factors that influence the roles and performance of APPs in team-based care and formulate strategies for effective integration into team-based care models.
Methods. This was a qualitative multiple-case study design. A total of 24 participants involved in team-based care were recruited. There were 8 cases, with participants divided into three groups per case: APPs, physicians, and healthcare leaders. Data collection was conducted by semi-structured Zoom interviews. Interviews were transcribed verbatim and subsequently imported into NVivo for initial coding. Data was analyzed using an iterative, reflexive, cross-case thematic analysis. Emergent themes were mapped onto the dimensions of the TEAM framework.
Results. Seven overarching themes emerged, demonstrating that APP integration is influenced by the interaction of organizational inputs, team processes, and outcomes.
Conclusion: Successful integration of APPs within team-based care calls for deliberate alignment of structural inputs, relational processes, and governance frameworks.
